• Your OS (left eye) and OD (right eye) prescription details
• Your PD (pupillary distance) measurement
• The doctor’s signature verifying the prescription
• The date the prescription was issued (must be current and not expired)

• Make sure all text is sharp and readable — blurry or cropped images may delay processing.
• Include the entire RX document, not just parts of it.
